This week, the Centers for Medicare and Medicaid Services (CMS) released their first ever risk-adjusted ranking for hospital readmissions for all skilled nursing facilities nationwide as part of their value-based care program. The goal of this ranking is to allow skilled nursing facilities to be judged on their relative performance while taking into account the specific kinds of patients the facility receives and treats.
Joshua Jacobs, Sr. Vice President of Windsor Healthcare, announced yesterday that "Merwick Care and Rehabilitation did really well, particularly in our region as well as compared to our peers. They ranked number 13 our of 357 skilled nursing facilities in New Jersey. He added, "we are hopeful that this externally verified data will be prove to be useful in advancing goals of our partner health systems as well as Accountable Care Organizations in the area."
Carolyn Bachonski, Director of Nursing at Merwick, said of the achievement, "this is truly something to celebrate with our team, as well as our Medical Director, Dr Sidhu, and our Nurse Practitioner Megan Bryne. They are so instrumental in driving our successes. This ranking helps us know we’re on the right track with managing the complex patient cases we receive in a manner that respects the hospitals goals and keeps these patients from getting readmitted."
